NURSING
NPLH, Inc. will provide medical case management to participants that receive residential supports from NPLH, Inc. and participants in the community that are DDA-funded and are in need of medical case management. Case management supports for participants receiving personal supports include, but not limited to: completion of the Health Risk Screening Tool; delegation of nursing duties (45-Day Nursing Assessments, Nursing Plans of Care, and Initial Nursing Assessments; scheduling appointments; and ordering medications.
In order to be a Delegating Nurse with NPLH Service Group, the candidate must have a BSN, must have a license to a Registered Nurse in the State of Maryland, and attend the mandatory Delegating Nurse/Case Manager training through DDA.
The responsibilities of the Delegating Nurse include, but not limited to:
- Delegation to NPLH Service Group’s trained, unlicensed staff;
- Conduct MTTP/CMT training for administration and staff of NPLH Service Group;
- Monitoring appointment compliance for participants receiving residential supports and personal supports (if applicable);
- Assist participants receiving residential supports and personal supports (if applicable) with advocating for their health with health care providers;
- Complete 45-Day Nursing Assessments for participants receiving residential supports and personal supports (if applicable);
- Complete Nursing Plans of Care for participants receiving residential supports and personal supports (if applicable) from NPLH Service Group;
- Complete Initial Nursing Assessments for participants receiving residential supports and personal supports (if applicable); and
Other duties outlined in COMAR 10.27.09 and COMAR 10.27.11.
