PERSONAL SUPPORT

NPLH Service Group will provide comprehensive personal supports to DDA-eligible participants  living independently or with their families. Supports include, but are not limited to:

  1. Nursing Case Management
  2. Transportation Supports
  3. Community Integration
  4. Assistance with budgeting
  5. Assistance with activities of daily living
  6. Completion of individual goals

NPLH Service Group will work to support adults with developmental disabilities, living independently or with their families, in living their best lives.
